AZOOSPERMIA—A DISCUSSION





What is Azoospermia?


Azoospermia is one of the most severe forms of male infertility. It is a



condition in
which a man has complete absence of sperm in his



ejaculate.



Is it a common problem?


It is not a common problem. It affects only 2% of general population. But



it is thought
that 10% -20%of men under treatment of infertility suffer



from Azoospermia.



What will be the cause for


Azoospermia?


Normally the sperm is produce in testicles, from the testes it passes



through
epidydimis, ductus deferens, seminalvesicle, ejaculatory duct



and urethra.From the
urethra the sperm to be implanted.So whatever the



problem in sperm production,transport or any infections
of male



reproductive organs will cause azoospermia. Let us briefly discuss about



it.
According to the cause azoospermia is divided in to two categories.



They are



1.obstructive cause.


2. Non obstructive cause.


Obstructive cause:


Obstructive azoospermia accounts for 40% of all cases of azoospermia.



The main causes are the following.



Genetic:


Genes may affect the sperm transport; the obstruction may be in


vasdeferens or
epidydimis. For Ex: congenital absence of vasdeferens.



Infections:


Infections male reproductive organs such as


  • Orchitis (infection of the testicles)

  • Prostitis (infection of prostate)

  • Balanitis(infection of glanspenis) etc.


Trauma:


Any trauma of male reproductive system including injury spine, lower



abdomen, pelvis, inguinal hernia repair may induce obstruction and cause


azoospermia.

Varicocele:

It is a condition in which the blood vessels of the scrotum, gets dilated


and
enlarged.


Vasectomy:

It is a surgical procedure of birth control in male. Vas deferens is a tube


carrying sperm from the testes to seminal vesicle it was cut or tied.


Nonobstructive azoospermia:

Drugs:

Drugs, such as steroids, antibiotics, and drugs to treat inflammation or



cancer
may affect male fertility.

Genetic:


Some genetic cause involve in non obstructive cause also. They are


Klinefelter's syndrome.


Kallmann's syndrome.


Hormones:


Disorders of the testicles may produce abnormal levels of hormones that



may
cause azoospermia.


Radiation:


Radiation, used for the treatment of cancer, may affect sperm


production.



Retrograde ejaculation
:


Retrograde ejaculation is a problem in which the semen will go inside the



bladder instead of coming out. It is due to spinal cord injuries, medicines,


diabetes
and neck of the bladder injury.


Other factors:

Other factors include



  • Smoking


  • Drinking alcohol


  • Pesticides


  • Heavy metals


  • Heat


  • Un descended testes (testicles that did not move from the abdomen)



How is azoospermia diagnosed?


Azoospermia can be diagnosed, when a couple came for infertility


treatment. Once azoospermia was diagnosed, They may need the following



investigations.



Physical examination:


During the physical examination, the physician can examine one’s external genitals



and also examine about Varicocele. The common signs of hormonal imbalance are,


Increased body fat, body hair, and breast tissue.The size and shape of your testicles


will also be examined.

Biopsy:


To determine the ability of the testicles to produce normal sperm.


Blood investigations:


Genetic screening:

Genetic testing may be done to look for abnormal genes. To determine the



abnormal genes, it may cause Problems with sperm production, sperm transport, or



formation of the male reproductive organs.


Imaging tests:

Imaging tests may include the following:



  • Magnetic resonance imaging scan:


  • Spermatic venography:


  • Ultrasonography:


Semen analysis:

To check a man's fertility


Urinanalysis:

To check retrograde ejaculation.


Azoospermia treatment:

It can be treated according to the cause.

Medicines:


Antibiotics may be given to treat an infection of the reproductive system.


Hormones are used to treat a hormonal imbalance.



Percutaneous embolization:

This procedure is for Varicocele.


Sperm extraction:

It should be more useful for obstructive case. The Sperm may be extracted


(removed) from the testicles or epidydimis. It can be extracted by following


methods.



MESA (Micro Epididymal Sperm Aspiration), in which sperm is removed from the


epididymis.


PESA (Percutaneous Sperm Aspiration), in which sperm is taken from directly from


the epididymis.


TESE (Testicular Sperm Extraction), in which a small tissue sample is taken from the


testicles in order to retrieve viable sperm.


Surgery:

It is also needed for obstructive cause.


How will you find support when you have

azoospermia?

Having azoospermia may be hard for each and every individual. One may feel angry,

depressed, or frightened. These are all normal feelings. Freely talk to your


caregiver,family, or friends about your feelings. You may need support from them


to share your feelings.


My experience about azoospermia



  • In my clinical practice treating azoospermia is a challenging and interesting one.But it was very difficult for me to manage the patient’s feelings.


  • All the patients are mentally depressed and frightened about the future. So they need more care and expect more love from their family members.


  • Treating them and teaching them about their medical problems are important in my practice.


  • Treatment for azoosperma is a step by step procedure according to every patient condition and cause.


  • The procedure should be explained to all the patients and their family members.


  • For all the patients routine investigation were done properly and they are adviced to maintain diet regimen and pathiyam.


  • If there is any surgical cause,a secondary opinion was taken from a surgeon.


  • Most probably 3-6 months period may be needed to treat azoospermia. For non obstructive causes such as infections can be treated successfully.


  • In the treatment of azoospermia, we need not only the patient’s cooperation but expect full suport from the all family members.


HAVING AZOOSPERMIA IS NOT A SIN FOR THE INDIVIDUAL,WE

HAVE TO SUPPORT THEM MORALLY AND MENTALLY!

ROLE OF SIDDHA MEDICINE IN BRONCHIAL ASTHMA


What is Asthma?


Chronic inflammatory disease of the airways followed by wheezing, shortness of breath,

cough and tightness of chest.

What are the symptoms of asthma?


  • Frequent Cough especially at night.

  • Short ness of breath.

  • Sleep disturbance.

  • Feeling tired.

  • Whistling sound during exhaling.

  • Recurrent infections like sneezing, running nose, sore throat etc.

  • Difficulty in speaking.

What are all the triggers of asthma?


Asthma, probably due to genetic and environmental allergic factors. Following are the

triggers of asthma.



  • Allergies due to various causes, like house dust, pollens, dander, feathers, hairs of pet animals, perfumes, chemicals, etc.

  • Smoke.

  • Respiratory infections.(viral and bacterial)

  • Physical exercise.

  • Cold air.

  • Emotions and stress.

  • Changes in the weather.

  • Gastro esophageal reflux disease.etc.

Complications of asthma:


  • Status asthmaticus.(repeated bouts of asthma attacks without interval)

  • Secondary infections of the lung.(Ex: tuberculosis, bronchitis)

  • Emphysema.(abnormal enlargement of air spaces, distal to terminal bronchiole)

  • Asphyxia

  • Pneumothorax.

  • Cardiac arrest.

  • Esnophilia.

  • Respiratory failure.

  • Hospitalization.

  • Death.

If any hospitalization need for asthma?

  • Indicative signs of hospitalization in asthma are

  • Persistent cough.

  • Persistent sweating.

  • Cyanosis.(bluish coloration of lips, finger nails and tongue)

  • Fast heart rate .

  • Difficulty in breathing, breathing by rib and neck muscles.

  • Rapid respiration.

  • Severe wheezing.

  • Confused stage of mind.

In Siddha System of Medicine Asthma is defined as Eraipunoi or

swasakasam , which was characterized by



  • Tightness of chest

  • Difficulty in breathing

  • During breathing the sound is like ( குழல், யாழ்)

  • Cough without expectoration

CLASSIFICATION:


It is classified in to five catagories.They are


1.Vali eraippu


2.Iyya eraippu


3. Iyya vali eraippu


4.Mukkutra eraippu


5.Mel nokku eraippu


VALI ERAIPPU:


All the symptoms are followed by indigestion of food. The main symptoms are


  • Tightness of chest and wheezing .It can be cured easily.

IYYA ERAIPPU:


All symptoms are followed by sudden changes in the weather. The symptoms are


  • Severe cough

  • Rhinits

  • Sneezing

  • wheezing

  • Sweating

  • Dryness of the tongue


  • Sleep disturbances

It is also called as Tamaka swasam.



IYYA VALI ERAIPPU:


It is otherwise called as Vitchina Swasam.The symptoms are combination of Vali and Iyya

eraippu .


MUKKUTRA ERAIPPU:


It is otherwise called as Maha Swasam. The symptoms are


  • Tightness of the chest .

  • Sweating .

  • Difficulty in breathing.

  • Rigor.

  • Giddiness.

  • Abdominal distension.

  • Irregulariy in speech.

  • Body pain.


MEL NOKKU ERAIPPU:


It denotes the severity of asthma. In this condition immediate hospitalization should be

done. The symptoms are


  • Inability to breathe.

  • Dryness of mouth.

  • Inability to speak.

  • Breathing through mouth.

  • Tears in eyes.


MY CLINICAL EXPERIENCE:


  • I have come across lot of cases of bronchial asthma in my medical practice.


  • For all the patients routine investigations and proper history were taken.


  • Usually they contact me in the chronic stage, 80%of them, were had history of asthma for the past 10yrs.


  • Siddha Medicines were prepared especially according to each and every individual condition.


  • For all patients Siddha medicines, given under proper anupanam and pathiyam.


  • Response to Siddha medicine and relieve of symptoms are excellent during treatment.



  • All patients are advised to avoid dried fish, packed food, tinned food, and refrigerated food, bottle drinks etc.


  • All are advised to take fresh foods and healthy food to improve their immunity power.


  • After 40 days treatment, in acute stages, they got very good relief in symptoms and they have no episodes of wheezing. If it occur, it should be tolerable.


  • After 90 days treatment, 80% of them got successful results .


  • After that I advised them to take some preventive medicines with regular interval according to each individual condition and severity of symptoms.


  • The interval may be 3months once or 6 months once or yearly once 40 days according to their severity of the disease.


It is true in my practice, that Siddha medicine have a


Wonderful effect on bronchial asthma.

FACTS INSIDE –MALE REPRODUCTIVE SYSTEM


Male reproductive system consist of following

organs,

  • 2 testes

  • 2 epidydimis

  • 2 vas deferens

  • 2 spermatic cord

  • 2 seminal vesicle

  • 2 ejaculatory duct

  • 1 prostate gland

  • 1 penis


Normally testicles produce sperm, which is situated within

the scrotum. Scrotum is a pouch ,deeply pigmented

skin ,fibrous, connective tissue, smooth muscle. From the

testicles the sperm flows through epidydimis, vasdeferens,

seminal ducts. Epidydimis is a tortuous tubule formed by

combination of tubules at the upper pole of the

testis.Epidydimis leaves the scrotum as vasdeferens in the

spermatic cord. The spermatozoa, becomes more mature in

the epidydimis, vasdeferens.



There are two spermatic cord one leading from each testis, consists of




  • 1 testicular artery


  • 1testicular venous plexus


  • Lymph vessels


  • 1 ductus deferens


  • Nerves. They all are responsible for ejaculation.



Seminal vesicles:

They are two in numbers. It is fibro muscular pouch, lying on the posterior

aspect of the bladder. At its lower end each seminal vesicle opens in to short

duct, which joins with ductus deferens to form ejaculatory duct. They

secretes a viscous fluid which keeps the spermatozoa alive.


Ejaculatory duct:


It is formed by the union of ductus deferens and duct of seminal vesicle, it

passes through prostatic gland joins with Prostatic part of urethra, carrying

seminal fluid and spermatozoa to urethra.


Prostate gland:

It lies within the pelvic cavity in front of the rectum and behind symphysis

pubis, surrounding first part of urethra. It also Secretes a lubricating fluid ,

passes in to urethra.


Urethra:

In male urethra is a common pathway for urine and semen. It has three parts.

1. Prostatic part.
2. Membraneous part.
3. Spongy part.


Prostatic part:

It originates from the urethral orifice of the bladder and passes through prostate gland.

Membranous part:

It is the shortest and narrowest part of urethra. It extends from the prostate

gland to bulb of penis.


Spongy part:


It is otherwise called as penile urethra.


It lies within the corpus spongiosum of penis and terminates at the external

urethral orifice in the glans penis.



Penis:



  • It is the male copulatory organ.


  • It conducts sperm and urine.


  • It lies in front of the scrotum.


  • It has root and body.


  • Root is made up of a pair of crurapenis and bulb of penis.


  • Through the bulb the urethra passes.


Body:


It is covered by thin skin and it’s made up of three cylindrical masses of

erectile tissue and involuntary muscle. The erectile tissues are one pair of

corpuscavernosa and single corpusspongisoum. The spongiosum is

anteriorly expanded in a triangular shaped structure called glanspenis. Above

the glanspenis the skin itself folded in double layered, called prepuce. Penis

is supplied by autonomic nerves and somatic nerves.Parasympathetic

stimulation leads to engorgement with blood and erection of penis.



PHYSIOLOGY OF MALE REPRODUCTIVE SYSTEM:


Male reproductive organs are stimulated by gonodotrophic hormones from

anterior lobe of pituitary gland. Normally, testicles in the scrotum produce

sperm, from the testes it passes through epidydimis, ductus

deferens,seminalvesicle, ejaculatory duct and urethra. From the urethra the

sperm to be implanted in female vagina during coitus.Sucessful

spermatogenesis takes place at a temperature about 3*c lower than

normal body temperature.

SEMEN:

Semen is the thick sperm containing fluid which is ejaculated during coitus.

Its normal


Volume - 2.5to 5 ml.


Count :40to100 million/ml.


Motility- is about 60%.


P H-7.2to7.8.


It is made up of



  • Spermatozoa.


  • Seminal fluid, which facilitates motility.


  • Fructose which nourishes spermatozoa.


  • Lubricating fluid from prostate gland.


  • Mucus secreted by glands of urethra.



Problems with the production or flow of sperm may affect male fertility. With

treatment, such as medicine and sperm extraction, your infertility may be

resolved and your partner may conceive. Towards this article we all are well

known about the anatomical structure and physiological functions of male

reproductive system.

SIDDHA SYSTEM A GIFT FOR FEMALE INFERTILITY – REALLY PATHETIC EXPERIENCE OF UNLUCKY COUPLE.




I would like to share the pathetic experience of my unlucky patient.

\A couple with complaint of infertility consulted me in the middle of November

2008 at my clinic. They have been under continuous treatment for the same

since 10- years without fruitful results. History of Intra Uterine Death (IUD)

before 10 years was documented while history taking. Due to continuous failure

in previous treatments ,the couple terminated all medical treatments regarding

the same problem for the past 2 years. Age of the couple is 36 years for male

partner and 32 for female.

They want to rectify their fault as early as possible. They are very eager to have

a baby. Through routine investigation I found them to be normal. Their body

condition was within normal limits .I advise them to wait for another

3-6 months. They agreed and started siddha treatment. As per siddha literature

I prescribed particularly prepared siddha medicines for them with suitable

anupanam. Regular pathiyam were maintained as per need . After 2 months

treatment she got 40 days amennorhoea.The patient was in unexpected

pleasure. I undergo routine confirmative test and her pregnancy was confirmed.

Later I referred them to one of the famous Gynecologist in Karaikudi for further

management and treatment.

A few months later her husband came for consultation. I came to know that his

wife had P.I.H ,she is taking oral antihypertensive drugs. I advise them common

salt free diet ,and to be patient . At the end of 9month they got misunderstanding

due to some family problem, they were separated from each other. The lady was

very much affected . Of course family is important. But she is in term and she

should take care of her baby also. But their problem went beyond their control.

Her BP was not controlled . The baby died inside the uterus at 9 month.


A very small flower is crushed before it’s blossom! . I feel

very very bad. It is my dreadful, pathetic experience.

MY LITTLE BIT EXPERIENCE ABOUT HYPERCHOLESTEROLEMIA:




  • In my clinical experience I met several cases of hypercholesterolemia, about 80% of them were under the age group of 35-45yrs.


  • Treating them and teaching them is very interesting in my 13yrs of medical practice.


  • Before treatment I undergo routine investigation, through modern methods.


  • Today all our life style are changed. So our food habits also modified in certain styles.


  • During counseling I came to know their food habits, hobbies, and lifestyle.


  • I advise them to avoid fast food, and to improve physical activity.


  • I also advise them to take home made foods, plenty of fruits and vegetables and to avoid fried foods.


  • Usually most of them avoid morning breakfast; I incise them to take breakfast.


  • I also advise them some yoga postures to reduce the fat depots in the body.


  • Most of the patients got wonderful results within a short period and got the satisfied results within a short time, 3months.


  • I also advise them to take some preventive treatment with regular intervals, six months once for 48 days.


  • During treatment regular pathyiam and diet regimen were advise them to follow it.


  • I have very much confident in treating hypercholesterolemia in my clinic.


  • Really siddha medicine do a very good healing in hypercholesterolemia without any side effects.


  • Through this article I want to reveal that hypercholesterolemia
    is like a slow poison ,it is the main cause for all diseases. So dear viewers be aware of it.

INTRODUCTION:

Today our topic is about pyto medicine. What is phyto medicine? phyto

medicine is nothing but herbal medicine. Herbal medicine plays an important

role in our routine life. Specific parts of a plants like leaf flower, bulb, root,

rhizome , bark, fruit are dried and powdered for medicinal uses. Herbal

medicine refers to traditional practice, based on the plants. Today phyto

medicines are available as tablets, capsule ,liquid extracts etc.

AIM:


The ultimate goal of this presentation towards



  • Improving the knowledge about phyto medicine .


  • Improving survival age


  • Reducing the need for interventional procedure.


  • Improving the quality of life.


MATERIALS AND METHODS:





As I am a siddha physician. I choose some of the herbs for



analysis. I select the topic hyperlipidaeamia. Why should I choose



hyperlipidaeamia. In nowadays hyperlipidaemia is one of the common



problems seen in younger Generations. Really it is a threat to many



people. It is due to consuming products rich in saturated fats,



like whole milk dairy products, poultry, and egg yolk, fried foods.



It is one of the main cause of all CAD’s that is coronary artery



disease. We can prevent it, one thing we should remember always





”உணவே மருந்து மருந்தே உணவு”



Low cholesterol diet including low in saturated fat that is fat free diary

products, lean meats, fish, shell fish, skinless poultry, whole grain foods, and

green fruits and vegetables. A simple low-cholesterol diet is a big help to you,

but only if you observe and follow these guidelines regularly.



We all should remember that our health is in our hands.



HOW DANGEROUS CHOLESTEROL IS ?

Fats including cholesterol cannot dissolve in the blood or water. So they bind to the

lipoproteins to travel throughout the body. When there is too much of cholesterol, it

builds upon the arterial wall, causing narrowing of arteries, and impeded the blood flow.

That will cause many problems like heart attack (myocardial infarction), stroke

(hemiplegia), etc.


WHAT IS GOOD CHOLESTEROL? AND WHAT IS BAD CHOLESTEROL?

Lipids in the diet are digested and absorbed in to blood in the form of free fatty acid

and chylomicrons. Chylomicrons are nothing but fat, protein complex. It composed of

protein, phospholipid, cholesterol, triglycerides. Liver plays an important role in fat

metabolism. It breaks the chylomicron and other lipids in to LDL, VLDL, and HDL.


GOOD CHOLESTEROL:



  • It does not deposit on the arterial wall.

  • Will not block the blood flow


  • It will improve the blood flow and reduces the risk of Myocardial infarction and stroke.Ex:HDL


  • Mainly present in plant sterols.




BAD CHOLESTEROL :


  • It will deposit on the arterial wall .


  • It will block the blood flow.

  • It combined with other fats tends to get attached in the inner walls of the
  • arteries which will reduce the blood flow and sometimes the cells of the tissues are died due to anoxia and cause heart failure, myocardial infarction,and stroke.Ex:VLDL,LDL,TGL.

  • Mainly present in animal products


THERAPEUTIC USES OF PHYTOMEDICINES:



Phyto medicines are used in many fields. In medical field it plays an important role. Some of these are


  • Anti cancerous action


  • Anti diabetic


  • Anti oxidant


  • Anti cholesterol action


  • Antiseptic


  • Antispasmodic


  • Aphrodisiac


  • Hepatotonic


  • Nutrient


  • Laxative


  • Haemostatic etc.




ANTI CHOLESTEROL EFFECT OF SOME FOOD:



Many food contain anticholesterol effect. Some of these are


  • Linseed


  • Oatmeal


  • Soyaflour


  • Corn

  • Peanut


  • Kidney bean


  • Rye


  • Wheat


  • Barley


  • Wheat bread


  • . Oliveoil


  • Flax seed oil


  • Fish oil


  • Black soyabean.






SOME OF THE HERBS CONTAIN ANTI CHOLESTEROL EFFECT:





Many herbs have anticholesterol effects. Some of these are





  • Allivum sativa


  • Avaena sativa


  • . Brassica oleraceae


  • Cucurbita pepo


  • Fagopyrum esculentum


  • Glicine max


  • Helianthus anus



  • Phaseolus acutifolius


  • Pisum sativum


  • Solanum tuberosum

From the above herbs I choose soya bean and garlic.

ABOUT SOYA AND GARLIC:



GARLIC:



We use garlic in day today life.





  • It contains allicin –alkaloid


  • Allyl propyl disulphide ,ajoene, Diallyldisulphyide,dithiin,Phyticacid,enzymes, vitamin, proteins, minerals, saponins, flavonoids, etc.


Actions :





  • Antiseptic


  • Antibiotic


  • Antimicrobial


  • carminative ,


  • stomachic,


  • Tonic,


  • stimulant


  • Diuretic


  • Expectorant,


  • Anthelmintic


Uses:

  • We can use garlic in many ways .

  • Garlic prevents heart diseases, including atherosclerosis, high cholesterol, high blood pressure, and cancer.

  • It is also used for common cold and throat infection.

  • It is also used to regulate the blood sugar level and to prevent the complication of Diabetes.

SOYABEAN:


  • Contains Glycine –Amino acid


  • Six kempferol glycoside


  • Butanol.


  • Soybeans also contain isoflavones, genistein, and daidzein and types of phytooestrogen.


  • It also contain unsaturatsd fattyacid omega-3- fatty acid and a- linolenic acid.


USES:



  • The kempferol glycoside relaxes the artery rings.


  • 200 ml of soya drink /day for 8 wk. Before test and after the drink the cholesterol level is noted. There is 7% reduction is noted.


  • The meta-analysis says that soya protein, significantly decreases in serum cholesterol, LDL(bad cholesterol) and .


  • Isoflavones content of soyabean will reduce the post menopausal problems in women.
    It is also used to prevent cancers of the breast,uterus or prostate.

CONCLUSION :

Herbal medicine cure hypercholesterolaemia and various diseases .Always

prevention is better than cure.So dear colleagues from today itself you start

taking garlic in food. Once again I thank you for Director of the board and
